Special Educational Needs and Disabilities (SEND): School Offer
Inclusion in Our School
In the Spring term of 2024, all of our school stakeholders worked together to build a shared definition of inclusion in our school. This is what they decided:
“At Captain Webb Primary School, inclusion means we are always changing our school and the way we work so that everyone in our community can be present, participate and achieve. We work hard to reduce exclusion from the learning, culture and community of our school. It involves the constant restructuring of our cultures, policies and practices so they respond to the changing diversity of students.”
At Captain Webb Primary School, we are committed to ensuring that all children, including those with SEND, are valued, included and able to thrive. We aim to remove barriers to learning and participation so that all pupils can learn, achieve, belong and thrive alongside their peers.
Our main aim is to provide an inclusive curriculum through high-quality adaptive teaching that takes account of individual needs and enables all pupils to access all areas of learning. We believe this can be achieved through careful planning, effective teaching strategies and, where appropriate, advice and recommendations from external professionals. We recognise that every child is unique and learns in different ways, and we adapt our teaching accordingly.
Our school offer highlights the provision available for pupils with SEND and links closely to our SEND Policy and SEND Information Report.
Partnership for Inclusion of Neurodiversity in Schools (PINS)
Captain Webb Primary School has participated in the Partnership for Inclusion of Neurodiversity in Schools (PINS) project. Through this work, we have strengthened our whole-school approach to neurodiversity by promoting early identification, staff development, collaborative working with families and the removal of barriers to learning, participation and belonging. This supports our commitment to ensuring all children feel valued, included and able to learn, achieve and thrive within a mainstream school environment.
Identification
Our SEND Policy outlines how children are identified and closely monitored when concerns are raised regarding a special educational need or disability. This follows
guidance from the SEND Code of Practice (2015) and links closely to the Telford and Wrekin Local Offer.
Any concerns regarding your child should initially be discussed with the class teacher, who will liaise with the SENDCo where appropriate. The class teacher and SENDCo can then meet with parents and carers to discuss how the child is being supported and agree next steps.
Graduated Approach and Intervention
Early identification is a key part of our approach. Ongoing assessment takes place throughout Day Care, Nursery, Reception and beyond, enabling additional support to be put in place whenever it is required.
We follow the graduated approach outlined in the SEND Code of Practice:
Assess
Teachers and school leaders identify concerns through ongoing assessment, observations, discussions with parents and progress monitoring.
Plan
Staff, parents and pupils work together to identify outcomes and the support required to help the child make progress.
Do
Support is implemented through high-quality adaptive teaching and, where appropriate, additional interventions.
Review
The effectiveness of support is reviewed regularly, and next steps are agreed with pupils and parents.
High-quality adaptive teaching remains the first response to meeting the needs of pupils with SEND. Additional interventions and targeted provision are used where necessary to support progress.
The Four Areas of Need
At Captain Webb Primary School we follow the SEND Code of Practice (2015), which identifies four broad areas of need:
· Communication and Interaction
· Cognition and Learning
· Social, Emotional and Mental Health
· Sensory and/or Physical Needs
We recognise that children may have needs across more than one area and that these needs can be interrelated.
The Telford and Wrekin Local Offer outlines the resources and services available to support children and young people with SEND and their families.

Pupil Participation
At Captain Webb Primary School, we believe it is of utmost importance that children are involved in decisions about their education and support.
Children are encouraged to share their views, contribute to target setting and review their progress. Pupils on the SEND register may complete a one-page profile which helps staff understand what is important to them, what they find difficult and how they can best be supported.
Pupils also have opportunities to contribute to wider school life through the School Council and other pupil leadership opportunities.
Curriculum Entitlement
The SEND Code of Practice states that all children and young people are entitled to an education that:
· enables them to achieve their best
· helps them become confident individuals living fulfilling lives
· prepares them for adulthood and future success
At Captain Webb Primary School, we strive to ensure that all children can access a broad and balanced curriculum. High-quality adaptive teaching and ongoing assessment help teachers identify barriers to learning and make appropriate adaptations.
Additional support may include:
· In-class support
· Small group interventions
· Individual support
· Adapted resources and learning materials
· Specialist equipment
· Nurture and wellbeing support
· Advice from external professionals

· Learning Support Advisory Team (LSAT)
· Shropshire Speech and Language Therapy
· Shropshire Occupational Therapy

The SENDCo regularly attends network meetings and liaises with colleagues across the local area to ensure practice remains current and effective.
Transition Arrangements
When pupils join or leave Captain Webb Primary School, we work closely with families and previous settings to ensure a smooth transition and a clear understanding of individual needs.
When moving between classes, children participate in transition days and have opportunities to become familiar with their new classroom and teacher. Additional transition opportunities may be arranged where required.
One-page profiles and transition information may be shared to help staff understand how best to support pupils.
Transitions to secondary school involve close collaboration between Captain Webb Primary School and the receiving school. Relevant SEND information is shared and discussions take place to ensure needs are understood and appropriate support can be planned.
For pupils with an Education, Health and Care Plan (EHCP), the SENDCo from the receiving school is invited to the Year 6 annual review to support effective transition planning.
